Faculty Proctored Test Request

All proctored testing requests must utilize these sessions for ASC to participate.

The nine sessions are scheduled to anticipate the timing of requests throughout the course of a semester.

Other conditions to note:

  • There will be a 3 hour maximum for test completion
  • Proctored test requests are to be used to assist students with singular instances of make-up test attempts.  These sessions are not to be used to accommodate multiple students from the same class.
  • The student will be required to pay a proctoring fee of $20.00 per exam
  • Requests for proctoring exams or quizzes which can be delivered using Blackboard will be declined
  • For proctoring needs which fall outside the scheduled sessions, ASC staff kindly request that faculty make arrangements within their respective departments.

Requesting an Exam attempt

  1. Proctored test requests are to be used to assist students with singular instances of make-up test attempts.  These sessions are not to be used to accommodate multiple students from the same class.
  2. Complete and submit the Proctored Exam Request form.   Be sure to enter the student’s email correctly, as well as the test conditions.  ASC adheres strictly to your conditions as they are entered.
  3. The student will receive an auto-generated email which will contain:
    1. a confirmation of the request
    2. the testing conditions as entered in the form submitted
    3. a link to the page to book appointments. Students will choose a session by filling out an online sign-up form. ASC will decline a student’s exam appointment if the information requested in Step 2 from the faculty member is not completed.
  4. If you do not upload the test materials with your request form, please submit the exam materials at least 24 hours prior to the student’s scheduled exam in the manner you indicate on the Proctored Exam Request form.
  5. Once the student has booked his/her appointment, you will receive an email with the student’s name and exam day/time. Should the student fail to arrive, cancel, or reschedule, you will receive email notification to your @laverne.edu email address.
