The university and SOS expressly disclaim any and all responsibility for any problems that may arise with regard to utilizing these services.<\/p>\n<p>&nbsp;<\/p>\n<h4><strong>Tri-City Mental\u00a0<\/strong><b>Health Services<\/b><\/h4>\n<p>Tri-City Mental Health is a public agency serving the diverse communities of Pomona, Claremont, and La&nbsp;Verne. Tri-City is dedicated to helping families and individuals of all ages reach their full potential. Available services include but are not limited to psychotherapy, clinical case management, medication support, peer-to-peer support, psychoeducation, linkage and referral, vocational training and support, socialization activities, and community outreach.<\/p>\n<p>The Tri-City resources below are free of cost.<\/p>\n<p>For additional information please visit the <a href=\"https:\/\/www.tricitymhs.org\/\">Tri-City Mental Health Website<\/a>.<\/p>\n<h5>Wellness Center<\/h5>\n<p>The Wellness Center is a hub of community activities that promote recovery, resiliency, and wellness for people confronting mental health issues. Staff located at this site, including peer advocates, family members, clinical staff, and others, provide a range of culturally competent, person- and family-centered groups, workshops and socialization activities designed to promote increasing independence and wellness for people of all ages.<\/p>\n<p>Services for young adults include self-help groups, peer support, independent living skills coaching, job vocational training, recreational activities, specialized supports, assessment and linkage, and leadership development.<\/p>\n<p>Services for adults include assessment and linkage, recreational and cultural activities, educational resources, services to promote independence, and support networks.<\/p>\n<p>1403 N Garey Ave Pomona 91767<\/p>\n<p>(909) 242-7600<\/p>\n<p>&nbsp;<\/p>\n<h5>Peer Mentors<\/h5>\n<p>Volunteer mentors receive specialized training and on-going weekly supervision and support from Tri-City clinical staff members. The volunteers meet individually with mentees and\/or in groups to provide support. Groups focus on providing support and creating opportunities for members to engage in projects that serve their communities and other wellness activities.<\/p>\n<p>(909) 326-4690<\/p>\n<p>&nbsp;<\/p>\n<h5>Supplemental Crisis Services<\/h5>\n<p>Supplemental Crisis Services are after-hours clinical support available to any Tri-City resident who is not enrolled in formal treatment services. The trained and experienced therapists who staff this crisis response line are available Monday through Thursday from 8 pm to 4 am, and during the weekend from 8 pm on Friday to 4 am on Sunday morning. If you are experiencing significant emotional distress and need assistance during these hours, call (909) 623-6131.<\/p>\n<p>During Tri-City business hours, and on a walk-in basis only, Supplemental Crisis Services also provides crisis and intervention for persons in Tri-City area who are not enrolled in formal treatment services. Staff that are trained and experiences in crisis assessment and intervention are available to assist persons experiencing significant emotional distress at 2008 North Garey Ave, Pomona, Monday-Friday from 8:30 am-4:30 pm.<\/p>\n<p>&nbsp;<\/p>\n<h5>Intensive Outreach Engagement Team<\/h5>\n<p>The Intensive Outreach Engagement Team is an experienced team of mental health professionals capable of responding to requests for assistance of intervention with any mental health issues. Issues can range from serious emotional crisis to seeking guidance for mental health support. This team will go to any location in the Tr-City area, such as a home, school, shelter, hospital or other community setting.<\/p>\n<p>Services provided are as follows: Assessment- assessing the situation and needs then working to identify what has contributed to the crisis. Intervention- provide brief, therapeutic interventions to stabilize the situation. Referral- assist in connecting individuals to treatment services for ongoing mental health support and\/or community resources. Follow-up- Follow-up with individual to ensure a smooth transition to any support or services that are being received. Community outreach- Raising community awareness of mental health issues and how IOET can support individuals in their path to recovery. Available for consultation to interested community members.<\/p>\n<p>(909) 623-6131<\/p>\n<p>&nbsp;<\/p>\n<h5>Community Navigators<\/h5>\n<p>The Community Navigator Program consists of highly trained individuals who specialize in linkage and referral to local resources. Navigators help people gain access to the services and support they need, both formal and informal.<\/p>\n<p>(888) 436-3246<\/p>\n<p>&nbsp;<\/p>\n<h4><strong>Telecare CSU Merrill Center<\/strong><\/h4>\n<p>This walk-in clinic is available to individuals 24 hours a day. Individuals who are feeling depressed, scared, anxious, angry, or stuck can visit the walk-in clinic to receive support. Services available are as follows: crisis assessment and evaluation for behavioral health and substance use disorders, brief crisis interventions utilizing evidence-based skill sets, coordination of admissions to inpatient facilities when necessary, peer mentoring, coordination of care with individual&#8217;s support systems and community providers, referrals and linkage to community resources.<\/p>\n<p>14677 Merrill Ave Fontana 92335<\/p>\n<p>(951) 643-2340<\/p>\n<p>For more information, Please visit the <a href=\"https:\/\/www.telecarecorp.com\/merrill-center\">Telecare CSU Merrill Center Website<\/a>.<\/p>\n<h4><strong>Project Sister Family\u00a0<\/strong><b>Services<\/b><\/h4>\n<p>Project Sister Family Services is the foremost agency dedicated to providing services to the women, children and men survivors of sexual assault and abuse and their families in the East San Gabriel and Inland Valley in Southern California. Project Sister Family Services pursues a two-pronged approach to addressing the problem of sexual assault and child abuse: providing services to those in crisis with extensive follow-up treatment, and specific sexual assault outreach and prevention education programs.
