ULV’s Online GI Bill\u00ae Enrollment Certification Request (of your classes) Form<\/h3>\n

For all VA benefit users!\u00a0 Fill out, upload your VA benefit required documents, and submit your GI Bill\u00ae Enrollment Certification Request Form (of your classes)<\/strong><\/a> <\/span>immediately following every time you register for, or add or drop classes.<\/em>\u00a0 You will need a list of your classes and will need to submit a separate form for each class with different beginning\/ending dates: Semester, Session I, Session II, January Inter-term, and Summer Session.\u00a0 Documents may include the first page of the VA’s Certificate of Eligibility (CoE), DD214, or other proof of veteran ID documents.\u00a0 Delayed submission will cause delayed payments of tuition, supplies stipend, and MHA.<\/p>\n

ULV’s Online Form Yellow Ribbon Request<\/h3>\n

Yellow Ribbon can only be used while you are using Post 9\/11, Chapter 33, and if you are rated at 100%.\u00a0 It pays for the full cost of tuition and fees above the VA’s yearly tuition cap.\u00a0 If you meet the requirements please submit this form once each year Application for Yellow Ribbon<\/strong><\/a><\/span> (link).\u00a0 If you have any questions regarding award notifications please email stuaccts@laverne.edu<\/a>.<\/span><\/strong><\/p>\n

VA’s Online form to apply for or update your VA Education Benefit, Change of Program\/Place Form VBA-22-1995, and to Request an Updated VA Certificate of Eligibility (CoE)<\/h3>\n

If you are a new benefit user, took a leave of absence, or are changing programs or schools, completing this form allows you to complete the items listed above.\u00a0 Link:\u00a0 VA.Gov’s “Apply For or Update Your Benefits<\/strong>“<\/a><\/span>\u00a0\u00a0 Note<\/strong>: The VA will not make payment without this form if you have changed colleges or programs.<\/p>\n

Chapter 31 Veteran Readiness & Employment (VR&E) Academic Worksheet, Education Plan, and Cost Analysis (for approval to attend ULV)<\/h3>\n

Once accepted into the VR&E program,\u00a0 you should discuss your options with your VR& E Counselor.\u00a0 Your VRE Counselor will need an Academic Worksheet and full Education Plan that shows prerequisites, (concentrations if a graduate student), as well as cost estimates from your program.\u00a0\u00a0 Email a request to the Military-Connected Advising Team at advisingmilitary@laverne.edu<\/strong><\/a><\/span> to complete both documents and once completed, you should forward all to your VR&E counselor. If approved, your counselor should issue a Tungsten Authorization\/PO Number, separately, to both your University\u2019s Certifying Official and the University (Follett) Bookstore. \u00a0NOTE: <\/strong>\u00a0An unofficial transcript must be submitted to your VRE counselor at the end of each semester.\u00a0 Unofficial transcripts can be found on your ULV Portal.<\/p>\n

Chapter 31 VR&E Text Books\/Supplies Information and Instructions<\/h3>\n

All Chapter 31 VRE books and supplies can be accessed through the University of La Verne Campus Store.\u00a0 Students should secure books\/supplies a minimum of 3 weeks prior to each semester, session, or Inter-term.\u00a0 Please read the online ordering instructions here: Bookstore Instructions<\/span><\/strong><\/span><\/a><\/p>\n

If the Bookstore response indicates that the VRE\u00a0 Tungsten PO has not been received by <\/span>Follett,\u00a0 please send your VRE Counselor the following in an email:\u00a0 “Follett and ULV are two separate accounts, please email my Tungsten PO to: va_campusstore@laverne.edu<\/strong><\/a><\/span>.<\/strong> Facility Code: 31801005 (La Verne). Tax ID: 36-2593135 (Follett).”\u00a0<\/span><\/p>\n

Chapter 31 VR&E Approval for Reimbursement Form<\/h3>\n

For one-time purchases of additional necessary equipment, books, software, and other items if noted as mandatory in the course syllabus.\u00a0 NOTE:\u00a0 With approval of your VRE Counselor you may be able to order these items via the ULV Bookstore.\u00a0 Print and fill out a VA form 28-1905M, take it to the University Certifying Official for signature,e and then submit the form with a scanned copy of receipts to your VRE counselor. Chapter 31 VR&E Pell Advance Fund Request Policy, and Form with Instructions<\/h3>\n

Undergraduate Veterans utilizing VRE benefits (Chapter 31) who will also receive a Pell Grant, may be eligible to apply for an advance on the credit expected to remain after tuition and fees are paid by the VA, up to $2,500.\u00a0 Additional information and instructions are located here<\/strong><\/a><\/span>.<\/strong><\/p>\n

Parent School Letter Request for Post 9\/11 and VR&E<\/h3>\n

Complete the online Parent School Letter Request<\/a><\/strong><\/span> if you wish to enroll in courses at another institution for transfer credits to your La Verne degree program. All coursework completed at any non-ULV institution must be submitted within 45 calendar days of the end of the full term\/semester where the coursework was completed.<\/p>\n

Active Duty<\/h3>\n

ULV\u2019s Online Military Tuition Assistance Authorization (TA) Voucher Submission<\/h3>\n

Submit your Request for Active Duty T.A. Authorization Voucher<\/strong><\/a><\/span> for each semester\/term you plan to use T.A. You must be admitted to a degree program and be enrolled in classes prior to submitting a TA voucher using this form. Students must contact your branch education center to apply for and receive an approved payment voucher. Please see (https:\/\/laverne.edu\/veterans\/tuition-assistance\/<\/a>)<\/span><\/strong> for details.<\/p>\n

ULV’s Online GI Bill\u00ae Enrollment Certification Request (of your classes)<\/h3>\n

Active duty students who utilize their Montgomery GI Bill\u00ae Active Duty or Post 9\/11 GI Bill\u00ae to pay tuition in a Session or Semester, must submit a VA Enrollment Certification Request<\/strong><\/a>.<\/span> You must be admitted to a degree program and be enrolled in classes prior to submitting this form. This option can be utilized for course charges that are not paid by the Armed Forces (TA), or for which you do not have an alternate funding source. This process is referred to as “Top-Up”.
