Class Visit Request Form Please use this form to request an ASC tutor to visit your class. Upon receipt of this form, a member of the Academic Success Center’s administrative staff will contact you to discuss the details of your request.
Workshop Request Form Please use this form to request ASC workshops. Upon receipt of this form, a member of the Academic Success Center’s administrative staff will contact you to discuss the details of your request.
Referral For Services The Referral for Services Form allows La Verne faculty or staff to identify students in their class who may be in danger of falling behind or failing a class and would benefit from the assistance of the Academic Success Center. Faculty and staff can identify difficulties, deficiencies or skills which are not meeting course or university expectations.
Service Request Form The Service Request Form allows both La Verne affiliates and local community organizations to request a variety of services and support from the Academic Success Center. Use this form to request classroom visits by ASC staff, use of any ASC spaces (including CC216 computer lab), tours of the ASC, development of specialized presentations or workshops, reserving or organizing ASC services for your class or group, academic advisor training, or to recommend a tutor to the ASC. The Academic Success Center is always looking for ways to support student and community success. Please note: submission of this form does not guarantee services. Submission will be reviewed by the ASC Director for approval.
Email Etiquette: Student writing to Instructor/Professor This downloadable handout could be a nice addition to your syllabus. It helps students to understand the conventions of respectful email correspondence expected of them, which is a different style than the the casual and brief approaches of Text Message and Twitter.
