This page is for students who have already been approved for accommodations through Accessibility Services. Below, you’ll find key responsibilities, important forms, and helpful resources to support your continued access to academic and campus life.
Student Responsibilities
- For academic accommodations, provide your accommodation letter to your instructors each term/semester, ideally at the beginning of the course or as soon as it becomes available. For housing accommodations, provide your accommodation letter to Housing staff (housing@laverne.edu).
- Notify us of any changes in your disability status, needs, or barriers that arise.
- Contact our office as soon as possible if you experience challenges or delays with accommodations being implemented.
Forms for Approved Accommodations
Exam Request Form
Students approved for testing accommodations must complete this form to schedule exams or quizzes at the Accessibility Services Testing Center (2150 1st Street, La Verne, CA 91750). A separate form must be submitted for each exam, and requests must be made at least five business days in advance to ensure proper arrangements.
Peer Notetaker Request Form
Students approved for peer notetaking accommodations can use this form to request a note taker for a specific class. A separate form must be submitted for each course where support is needed. After submitting the form, students will receive confirmation details within 1–2 weeks.
Alternate Media Request Form
Students approved for alternate media accommodations may use this form to request accessible versions of their textbooks. Requests are processed in the order received, and missing or incorrect information may cause delays. Students must provide proof of purchase for each book requested. After submission, Accessibility Services will notify the student within three business days with an estimated timeline for delivery, which may vary depending on publisher response times.
