This page is for students who have already been approved for accommodations through Accessibility Services. Below, you’ll find key responsibilities, important forms, and helpful resources to support your continued access to academic and campus life.
Student Responsibilities
- For academic accommodations, provide your accommodation letter to your instructors each term/semester, ideally at the beginning of the course or as soon as it becomes available. For housing accommodations, provide your accommodation letter to Housing staff (housing@laverne.edu).
- Notify us of any changes in your disability status, needs, or barriers that arise.
- Contact our office as soon as possible if you experience challenges or delays with accommodations being implemented.
Forms for Approved Accommodations
Required Policy Forms (to be completed before the Request forms below)
Before submitting a request, students approved for testing at Accessibility Services or alternate media must review and complete the policy acknowledgment forms below. These forms confirm understanding of expectations and are required before accommodations can be coordinated.
Exam Proctoring Testing Procedures (complete if approved for testing accommodations)

Request Forms
Once policy forms have been submitted, students may use the request forms below to initiate specific accommodations. These forms allow our office to coordinate services such as exam proctoring, peer notetaking, or alternate media in a timely manner.
Exam Request Form
Students approved for testing accommodations must complete this form to schedule exams or quizzes at the Accessibility Services Testing Center (2150 1st Street, La Verne, CA 91750). A separate form must be submitted for each exam, and requests must be made at least five business days prior to the exam date to ensure proper arrangements. While we will make every effort to accommodate your preferred exam time, there may be occasions where exams are scheduled slightly before or after the in-class exam time.
Peer Notetaker Request Form
Students approved for peer notetaking accommodations can use this form to request a note taker for a specific class. A separate form must be submitted for each course where support is needed. After submitting the form, students will receive confirmation details within 1–2 weeks.
Alternate Media Request Form
Students approved for alternate media accommodations may use this form to request accessible versions of their textbooks. Requests are processed in the order received, and missing or incorrect information may cause delays. Students must provide proof of purchase for each book requested. After submission, Accessibility Services will notify the student within three business days with an estimated timeline for delivery, which may vary depending on publisher response times.

Genio Notes (formerly Glean): Notetaking Assistive Technology
Genio Notes is a web-based and mobile note-taking tool available only to students who have been approved for this accommodation. Need help getting started? View helpful information and tutorials prior to using this tool in class.
Kurzweil 3000: Text-to-Speech Technology
Kurzweil 3000 is a free, web-based learning tool available to all University of La Verne students (regardless of accommodation status). It offers text-to-speech, note-taking, and study support features designed to make reading, writing, and learning more accessible and efficient—especially for students with learning differences. Academic Success Center (ASC)
The ASC, part of Libraries & Learning Commons in Wilson Library, offers a range of services to support students. These include tutoring, academic coaching, classroom support, 24/7 tutoring via Blackboard, faculty/staff assistance, workshops, student resource development, and placement testing services. Students experiencing academic challenges, time management issues, studying difficulties, or organizational problems should book an appointment with the ASC.
Student Outreach & Support (SOS)
SOS helps students during challenging times in their college experience, such as transitioning to college, mental health challenges, or academic difficulties. They offer support and resources through the CARE Network, case management support, and the Behavioral Intervention Team. If a student needs help or is concerned about a peer’s well-being, they should contact SOS through the online referral or via email at sos@laverne.edu.
