The ULV SHS is committed to protecting the health and well-being of the campus community to allow students to focus on educational and personal goals. To assist in keeping our campus healthy, the SHC has established immunization guidelines to protect the campus community from vaccine related diseases and outbreaks.
The SHC has partnered with MedProctor to assist with collecting immunization records. MedProctor is a secure online data collection application that will assist the ULV SHC in collecting all medical related documents: immunization records, immunization titers, physical exam forms, health/medical certificates, declination statements, and any other medical related documents.
Please follow the provided instructions for all students to upload the following documents (if applicable) through the MedProctor portal. To begin the process, the student must have their campus assigned email address, ending in ‘laverne.edu’.
Once enrollment has provided the student with their campus email address the student can sign up at MedProctor.com and upload documents. For assistance with campus email address, please contact enrollment services, admissions@laverne.edu.
