Incoming/Transfer Student Medical & Immunization Clearance

The ULV Student Health Center (SHC) is committed to protecting the health and well-being of the campus community to allow students to focus on educational and personal goals. To assist in keeping our campus healthy, the SHC has established immunization guidelines to protect the campus community from vaccine related diseases and outbreaks.

The ULV SHC is requesting all immunization documents and medical clearance forms to be submitted to the SHC. This includes all medical related documents: immunization records, immunization titers, physical exam forms, health/medical certificates, declination statements, and any other medical related documents. Please email these documents to the SHC, at studenthealthcenter@laverne.edu. Only authorized personnel has access to this medical information. It is a secure submission. To begin the process, the student must have their campus assigned email address, ending in ‘laverne.edu’. For assistance with a campus email address, please contact enrollment services, admissions@laverne.edu. Students may also drop them off in person at the SHC.

Documents will be reviewed by SHC staff. Please allow up to 4 weeks to be notified of clearance.

  1. Tuberculosis (TB) Risk Assessment Questionnaire Form
  2. Health History Form
  3. Meningitis Awareness Disclosure Form (all housing students))
  4. Religious Exemption Form (if applicable)
  5. Medical Exemption Form (if applicable)
