Medical History & Immunization Recommendations

Welcome to the University of La Verne Student Health Center

The SHC is conveniently located on the main campus at 2147 E Street, on the corner of E Street and Second street.  The mission is to foster academic success by supporting all students physical and wellness needs. The SHS staff providers are committed to delivering sensitive and confidential healthcare to the ULV diverse student population. The SHC is available to all actively enrolled students. Students can call the center to make an appointment at 929-448-4619. Appointments are available on a first come basis.

Immunization Record Submission

The CDC (Centers for Disease Control and Prevention) and ACIP (Advisory Committee on Immunization Practices) develops recommendations for United States immunizations. It is important to receive the scheduled immunizations or vaccinations. Being up to date with immunizations assures the ULV campus community is protected from vaccine related diseases and outbreaks that can lead to disabilities and/or death. The SHS is committed to protecting the health and well-being of the campus community to allow students to focus on educational and personal goals. It’s a simple and effective way to protect the campus.

The ULV recommends all student submit their immunization records. The SHC has partnered with MedProctor to assist with collecting these records. MedProctor is a free secure online data collection application that will assist the ULV SHC in collecting all medical related documents: immunization records, titers, physical exam forms, health/medical certificates, declination statements, and any other medical related documents.

All (freshman, returning, transfer, exchange, international, new enrollee’s) will receive an email from MedProctor inviting you to sign up and submit your documents. The documents are reviewed by MedProctor for acceptance according to the CDC and ACIP guidelines.

Documents are not submitted to the SHC.

TB Screening Questionnaire

The SHC supports the campus community course requirement for TB screening. The SHC aims to support educational programs and/or internships by offering free TB screening to everyone . Call the SHC to make an appointment for TB screening, 909-448-4619.  Please see  TB Risk Questionnaire form 24-25

Meningococcal Immunization

California Health and Safety Code 1.7, requires specific student and parent notification about Meningococcal disease.  Housing students are considered to be at a higher risk of communal spread of diseases. The CDC and ACIP recommends that college students living in residence halls have 2 meningococcal vaccinations. For all housing students, immunization records are reviewed for adherence to the Meningococcal immunization adherence by MedProctor. The immunization records are submitted to MedProctor for review. The parent and student are REQUIRED to complete the following document and submit it to MedProctor or the student health center prior to residing in on-campus housing.  California State Required Meningitis Awareness Disclosure Form
