End of the Year Report Requirements

Format of Departmental End-of-Year Report is based on the individual style (prose, bullets, etc.) of the director. The purpose of the end-of-year report is to allow the department’s director and staff to reflect on the following AND include the following:

  1. Established goals for the year
    • Accomplishment or lack of accomplishing the goals
    • Action plans reviewed
  2. Highlights for the year
  3. Challenges for the year
  4. Issues that need to be addressed
  5. Preparation plans for establishing new goals
  6. Mission Statement
  7. Vision Statement
  8. Departmental Strategic Plan: Outline your departmental strategic goals for the next 5 years
  9. Organizational Staff Chart (include student staff)
  10. Current PDQ’s for all staff positions: Include job descriptions for Interns and Student Employees*
  11. Diversity Statistics (e.g. how many women vs. men do you serve each year?)
  12. Curriculum Details / Syllabus: (e.g. Career and Life Planning, LEAD Classes)
  13. Detail how your department has collaborated with faculty and in what ways? (e.g. presentations, workshops, speakers, etc.)
  14. List Staff Development activities you and your staff have participated in during the year.
  15. Summarize how departmental programs and services are meeting ULV Mission Statement and the 4 pillars.
    1. Values Orientation
    2. Community and Diversity
    3. Lifelong Learning
    4. Community Service

Reports are traditionally due on or about June 15 (mid-June). 


* All staff PDQ’s should be evaluated each year during the employee’s Personnel Evaluation Review.
