Social Justice Reporting Form

Use this form to report incidents of social injustice or discrimination (these may be acts that promote hate, fear, intimidation, unfair treatment, or oppression against an individual or a group) across gender, sexual orientation, race, color, age, creed, national or ethnic origin, physical or mental disability, veteran status, pregnancy status, religions, or other protected categories.

  • Reports will be received by the Office of Diversity & Inclusivity for follow-up and/or referral to Human Resources, CSAD, Campus Safety and/or Law Enforcement
  • Reports can be submitted anonymously

Student Outreach & Support

SOS is dedicated to assisting students who are:

  • In distress
  • Facing challenging life circumstances
  • Experiencing behavior or psychological health concerns that may be disruptive, harmful or pose a direct threat to themselves or to the health and safety of the university community

If you or a student you know is in distress and needs support, please submit a SOS referral.

Behavioral Intervention Team

BIT addresses students, faculty, and staff whose behavior or psychological health condition may be disruptive, harmful or pose a direct threat to themselves or to the health and safety of the university community.

If you encounter someone who is a concern, report the person to BIT using the SOS referral.

Sexual Assault Reporting (Title IX)

All university employees, except for designated confidential resources, are required to report any potential discrimination or harassment on the basis of sex, including sexual assault, intimate partner violence, and stalking, directly to the Title IX Coordinator.
