Banner Account Request Form

To request a banner account, please complete the following steps:

1. Request and complete the on-line FERPA Training Course. (To request the course be assigned to you, contact the Office of Human Resources at lavernehr@laverne.edu. Please print the completion certificate as verification)

   If you need Cyber Security Training people email the Office of Human Resources at lavernehr@laverne.edu to obtain the training.

2. Fill out the Banner Account Request and submit your FERPA certification here.
