Banner Account Request Form

To request a banner account, please complete the following steps:

  1. Request and complete the on-line FERPA Training Course. (To request the course be assigned to you, contact the Office of Human Resources at Please print the completion certificate as verification)

    If you need Cyber Security Training people email the Office of Human Resources at to obtain the training.

  2. Fill out the Banner Account Request and submit your FERPA certification here.
