Forms for Faculty: Study Tours and Travel Abroad

The forms downloadable below should be used by faculty to plan and lead responsible and safe study tours abroad. The Study Abroad Office provides these documents to faculty so that they have information needed to prepare for and implement their study tours abroad. The Risk Management waivers must be completed and accident reports filed. All Study Tours abroad must be approved by the faculty member’s academic department and college dean.
