1. Select Your Review Type
Before getting started, it is important to select your review type. Exempt applications are streamlined and require less information than expedited or standard reviews. Thus, knowing your review type is integral to preparing your application. Follow these steps to select the appropriate type of review: 1) use the decision charts from HHS to ascertain if you qualify for an exempt review available on the Does My Application Qualify as Exempt? page, 2) if you do not qualify for an exempt review, read the nonexempt policies and procedures to ascertain if your study fits better as an expedited or standard review.
2. What to Include with Your Application
Click on the What to Include link in the menu on the left to go over the types of materials you will need to attach to your application. These documents should be prepared and signed (if required) PRIOR to submission to the IRB; completed documents are REQUIRED in order to be reviewed. For exempt reviews ONLY, recruitment/site permissions and copyright permissions are not required.
3. Where to Submit Your Application
As of September 1, 2016 the La Verne IRB started using IRBManager for all IRB submissions (applications, amendments, extensions, and closure forms). As of September 15th, any applications or forms submitted via email were no longer accepted.
